Volunteers will make outbound phone calls to St. Joe's discharged patients. Patients are identified by the Transition Support Call Center nurses that require a "friendly phone call" to add some socialization in the life of a patient. Calls are of a non-clinical nature. Volunteers document any concerns and work together with the department to triage callers when necessary.
Volunteer Duties:
- Obtain call list from TSCC staff
- Clarify any questions pertaining to the patient
- Make friendly phone calls
- Use scripting on all calls
- Triage calls when deemed appropriate
- Keep conversations light and cheerful
- Debrief at the end of the shift
